604 Donation Request

Our 6'Ohana believes in giving back to our communities and we are honored to have those opportunities. We understand that our community needs are significant and we take your requests to heart. It's our goal to enhance our limited giving by carefully aligning with organizations that share the 604 spirit.

Please share how your cause can benefit from 604 by filling in the form below.

Consider the following guidelines:

Be specific when describing your function and how you would like us to participate.
Submit your request at least 8 weeks in advance.
No verbal requests will be considered. All requests must be submitted through this form.
Donation requests are limited to one per year.
Political organizations or politically associated events will not be considered.

Additional details:

If your request is fulfilled your organization must be able to provide an official documentation of 501(c)(3) status and written acknowledgement of the donation on official letterhead.
Share your success! We kindly ask that you share photos/videos to showcase the impact of the donation you received.
We try to evaluate all donation requests within one week. Please reach out to accounting@restaurant604.com if you have not heard back within that timeframe.
